Ordering your patient’s test is easy

Four steps to a full genomic story

Step 1

Request specimen kits

Request your specimen kits by calling Customer Service. Note that each order requires two samples (solid tumor tissue and matched-normal blood) which typically ship from different locations. Kit requests are usually processed within one business day.

Step 2

Place your order

Select a test screen in the Exact Sciences Provider Hub.
It’s easy to submit an order for the OncoExTra test directly through our Exact Sciences Provider Hub.

You can also place an order by downloading a test requisition form and faxing it to +1 602-682-5077.

Step 3

Collect and ship samples

Exact Sciences will work directly with the pathology team and the treating provider to obtain samples. Please ensure all primary specimen containers are labeled with two unique patient identifiers and the collection date. Ship samples in the supplied specimen kits.

Step 4

Receive test results

Most OncoExTra reports are available approximately 14 days from the date we receive both samples in our lab.1 Access the results through the Exact Sciences Provider Hub or receive the report by fax.
